1-on-1 Mentoring Informed Consent & Client Agreement

 Please review the following information carefully. By checking the required acknowledgment within the client intake form and submitting the form, you confirm that you have read, understand, and voluntarily agree to the following:

A Few Details Before We Begin 

Before we begin working together, please take a few moments to complete the client intake form and review the informed consent below. Your responses will help me better understand what is bringing you to mentoring, the areas of your life or spiritual journey you would like to explore, and how I can best support you throughout our work together.

MENTORING INFORMED CONSENT & CLIENT AGREEMENT

Please review the following information carefully. By checking the required acknowledgment within the client intake form and submitting the form, you confirm that you have read, understand, and voluntarily agree to the following:

1. Nature of 1-on-1 Mentoring

I understand that 1-on-1 Mentoring with Astralis Healing is a personalized spiritual mentoring and coaching service intended to support self-awareness, personal growth, spiritual development, intuitive exploration, healing, and the achievement of personally meaningful goals.

Sessions are individualized and may include conversation, reflection, spiritual guidance, intuitive insight, practical coaching, education, exercises, practices, or other approaches within the scope of Astralis Healing based upon my needs, intentions, and areas of focus.

I understand that mentoring is a collaborative process. Jordyn Kurtz serves as a mentor and guide, while I remain responsible for my participation, choices, implementation, and personal growth throughout the mentoring relationship.

2. Scope of Practice & Professional Care

I understand that 1-on-1 Mentoring is not psychotherapy, counseling, psychiatric treatment, medical care, diagnosis, or another licensed healthcare service.

Astralis Healing, LLC and Jordyn Kurtz do not diagnose medical or mental-health conditions, prescribe medications, provide medical or psychiatric treatment, or advise clients to discontinue or alter treatment prescribed or recommended by a licensed healthcare professional.

Mentoring is not intended to replace appropriate medical, psychological, psychiatric, legal, financial, or other professional care. I remain responsible for determining when assistance from an appropriately qualified professional is needed.

3. Spiritual & Intuitive Nature of Mentoring

I understand that mentoring through Astralis Healing may incorporate spiritual perspectives, intuitive guidance, energetic concepts, astrology, tarot or oracle cards, meditation, visualization, spiritual practices, or other complementary tools when relevant to my goals and the direction of our work together.

I understand that spiritual and intuitive information is inherently interpretive and should not be treated as objective fact, professional advice, or a guaranteed prediction of future events.

I remain responsible for deciding what resonates with me, what I choose to incorporate into my life, and what actions or decisions I make based upon our work together.

4. Client Participation & Personal Responsibility

I understand that meaningful mentoring requires my active participation and that results may depend upon factors including my engagement, consistency, willingness to reflect, implementation of practices or recommendations, personal circumstances, and decisions outside of sessions.

I understand that I may communicate my goals, questions, concerns, preferences, and boundaries throughout the mentoring relationship.

I remain responsible for my physical, emotional, psychological, spiritual, relational, professional, and financial decisions. Guidance provided during mentoring is intended to support my own discernment and decision-making rather than replace it.

5. Emotional Experiences & Personal Growth

I understand that personal and spiritual development may involve exploring beliefs, patterns, relationships, past experiences, emotions, behaviors, boundaries, identity, or other personally meaningful subjects.

I understand that this process may sometimes bring forward uncomfortable emotions, realizations, resistance, uncertainty, grief, frustration, or other emotional responses.

I understand that mentoring does not constitute mental-health treatment. If concerns arise that would be more appropriately addressed by a licensed medical or mental-health professional, I remain responsible for seeking appropriate professional support.

6. Communication & Professional Boundaries

I understand that scheduled mentoring sessions are the primary setting for individualized mentoring, coaching, spiritual guidance, and deeper personal processing.

Communication outside of scheduled sessions may be used for reasonable administrative matters, scheduling, clarification, or other communication permitted within the mentoring program, but does not create unlimited or on-demand access to Jordyn Kurtz.

I understand that response times may vary and that Astralis Healing does not provide emergency, crisis, medical, or mental-health services. If I experience an emergency or crisis, I understand that I should contact an appropriate emergency or qualified professional resource rather than rely upon mentoring communications.

7. Privacy & Confidentiality

Information I provide through my client intake form or share during an individual mentoring session will be treated with care and handled in accordance with the Astralis Healing Privacy Policy.

I understand that information may be disclosed where required by applicable law or when I have provided appropriate authorization.

Astralis Healing will not intentionally use identifiable information, private session details, photographs, recordings, testimonials, or excerpts from my individual experience for marketing, advertising, social media, educational, or other public-facing purposes without my permission or other appropriate authorization.

8. No Guarantees of Outcome

I understand that personal growth, spiritual development, healing, and life circumstances are individual and that no particular outcome, transformation, result, relationship outcome, financial result, spiritual experience, or other change is promised or guaranteed through mentoring.

I understand that Jordyn Kurtz cannot make decisions or create outcomes on my behalf. My results depend upon many factors, including circumstances outside the control of Astralis Healing.

9. Mentoring Package, Scheduling & Session Use

I understand that each 1-on-1 Mentoring session is approximately 60–90 minutes in length and that my mentoring package includes the number of sessions identified in the offer I purchased.

I understand that purchasing a mentoring package does not automatically reserve specific appointment dates or times. Sessions must be scheduled individually through the booking process provided by Astralis Healing and are subject to appointment availability.

I understand that the Intuitive Guidance Mentorship must be started within 12 months of the original purchase date, with my intake call marking the start of the program, and that once it begins, all sessions must be completed within 4 months of my intake call (the 3-month program plus a 30-day grace period). The Astralis Reiki Apprenticeship follows the timeline outlined in my Apprenticeship Agreement. Any sessions not used within the applicable timeframe will expire and will not roll over, be extended, refunded, credited, or exchanged for other Astralis Healing services except where otherwise required by applicable law.

I am responsible for scheduling and completing my sessions within the applicable 12-month period. Availability near the end of my package term is not guaranteed, and I understand that I should schedule my remaining sessions with sufficient time to use them before they expire.

Cancellation, rescheduling, late-arrival, missed-session, refund, and other applicable scheduling provisions are governed by the Astralis Healing Terms & Conditions, Privacy Policy, and Cancellation & Rescheduling Policy.

10. Voluntary Participation & Ending the Mentoring Relationship

I understand that my participation in mentoring is voluntary and that I may choose to discontinue the mentoring relationship at any time.

I understand that choosing not to continue participating does not automatically entitle me to a refund for services, sessions, or packages already purchased and that any applicable refund or cancellation terms are governed by the offer under which I purchased, the Astralis Healing Terms & Conditions, and applicable policies.

Astralis Healing also reserves the right to discontinue the mentoring relationship when appropriate, including in circumstances involving repeated boundary violations, inappropriate conduct, nonpayment, misuse of services, or circumstances in which continued mentoring is no longer appropriate within the scope of the service, subject to applicable law and contractual obligations.

11. Client Responsibility & Release of Liability

I understand that I am voluntarily choosing to participate in 1-on-1 Mentoring and accept responsibility for my participation and for how I interpret, integrate, and apply information or guidance discussed throughout the mentoring relationship.

I understand that decisions and actions I take before, during, or following mentoring remain my responsibility. I agree that I will not hold Jordyn Kurtz or Astralis Healing, LLC responsible for decisions or actions I take based upon my personal interpretation or application of mentoring, spiritual, intuitive, or educational information provided through the service.

To the fullest extent permitted by applicable law, I knowingly and voluntarily assume the risks associated with my participation and release and hold harmless Astralis Healing, LLC and Jordyn Kurtz from claims, demands, liabilities, damages, losses, or expenses arising from or related to my voluntary participation in mentoring, except to the extent that liability cannot lawfully be waived or limited.

12. Agreement & Acknowledgment

I acknowledge that my purchase and participation are also subject to the applicable Astralis Healing Terms & Conditions, Privacy Policy, and Cancellation & Rescheduling Policy.

By checking the required acknowledgment within the client intake form and submitting the form, I confirm that I have had the opportunity to review this Mentoring Informed Consent & Client Agreement, understand the information provided, have had the opportunity to ask questions, and voluntarily consent to participate in mentoring services provided by Astralis Healing, LLC and Jordyn Kurtz.
