Retreat Participant Consent, Health & Liability Waiver

Event Overview

This agreement applies to participation in any retreat hosted by Astralis Healing and Jordyn Kurtz, including but not limited to the April 16–19, 2026 Women’s Mountain Retreat in Banner Elk, North Carolina (hereafter “the Retreat”).

The Retreat may include, but is not limited to:

• Spinal Flow Technique sessions or demonstrations

• Energy healing and Reiki-based practices

• Yoga, meditation, and guided breathwork

• Somatic and nervous system regulation exercises

• EFT/tapping and movement

• Sound healing, journaling, and group workshops

• Hiking, nature immersion, and outdoor exploration

• Bonfire gatherings and fire ceremonies

• A combination of catered meals, snacks, and self-serve food options provided on-site

Participant Responsibility & Acknowledgment

By signing below, I acknowledge and agree that:

• My participation is completely voluntary and undertaken at my own discretion and risk.

• I am solely responsible for monitoring my own physical, emotional, and mental well-being.

• I may choose not to participate in any activity at any time without penalty.

• I understand that yoga, meditation, breathwork, energy healing, Spinal Flow, and somatic work may involve physical movement, emotional release, light touch, or breath-related sensations.

• I release Astralis Healing, Jordyn Kurtz, and any assistants, volunteers, co-facilitators, contractors, or affiliated parties from any liability related to injury, illness, emotional distress, loss, or damages before, during, or after the Retreat.

Health, Medications & Allergies

• I affirm that I am in adequate physical and emotional health to safely participate in all retreat activities.

• I am responsible for bringing and managing all personal medications, supplements, and health supplies.

• I understand that Astralis Healing and its facilitators do not provide or administer medications. Over-the-counter medications (e.g., Tylenol, Advil, allergy medications) will not be dispensed, and I am responsible for managing my own medical needs.

• I acknowledge that meals and snacks provided during the retreat may include a mix of catered food and self-serve options (such as fresh fruit, pantry items, or stocked groceries).

• I understand that while reasonable care will be taken to accommodate dietary preferences, Astralis Healing, Jordyn Kurtz, and any chef or caterer cannot guarantee an allergen-free environment.

• I am responsible for reviewing ingredients and choosing food that aligns with my dietary needs.

• I release Astralis Healing, Jordyn Kurtz, the chef, caterers, assistants, and venue owners from any liability arising from food allergies, sensitivities, or adverse reactions.

Travel, Lodging & Venue Disclaimer

• I am responsible for arranging and covering my own transportation to and from the Retreat.

• I acknowledge that the retreat venue and surrounding areas may include stairs, natural terrain, and other environmental factors that carry inherent risk.

• I release Astralis Healing, Jordyn Kurtz, and all venue owners, property managers, and Airbnb hosts from any responsibility for injuries, accidents, or losses that occur on the property or during travel to/from the retreat.

Assumption of Risk for All Modalities

I understand and voluntarily assume all risks associated with participation in retreat activities, including but not limited to Spinal Flow Technique, energy healing, yoga, meditation, breathwork, somatic practices, tapping, journaling, sound healing, fire ceremonies, hiking, and group experiences.

I acknowledge that these experiences may cause temporary physical, emotional, or energetic discomfort such as soreness, dizziness, fatigue, nausea, or emotional release.

I agree to inform facilitators immediately if I experience pain, distress, or any unsafe condition and will discontinue participation if necessary.

I understand that Astralis Healing and its facilitators:

• Are not licensed medical, psychological, or healthcare professionals;

• Do not diagnose, treat, or prescribe;

• Offer all sessions and activities as complementary wellness experiences only;

• Make no claims or guarantees of outcomes or specific results.

Emergency & Medical Care

• I authorize Astralis Healing and its representatives to seek emergency medical care if necessary.

• I understand that I am fully responsible for all medical costs incurred.

• I have provided accurate emergency contact information.

Code of Conduct & Confidentiality

• I agree to respect the privacy, boundaries, and personal processes of all participants and facilitators.

• I will not disclose or share any personal or sensitive information revealed during group or individual sessions.

• I agree to maintain respectful communication and behavior throughout the retreat.

• Astralis Healing reserves the right to remove any participant from the retreat if their behavior is deemed unsafe, disrespectful, or disruptive to the group or environment, without refund.

Cancellation & Refund Policy

• Retreat deposits & payments are non-refundable.

• Cancellations made after the final payment date remain the participant’s financial responsibility unless the spot can be filled by another registrant.

• Refunds will not be provided for late arrivals, early departures, or missed activities.

• In the event of retreat cancellation due to unforeseen circumstances (illness, weather, venue issues, etc.), Astralis Healing may issue a credit for future events but is not liable for additional expenses (e.g., travel, lodging, or airfare).

Indemnification & Third-Party Protection

I agree to indemnify, defend, and hold harmless Astralis Healing, Jordyn Kurtz, all facilitators, assistants, co-teachers, contractors (including chefs, yoga instructors, and Spinal Flow practitioners), and any venue owners or property managers from any and all claims, damages, costs, or expenses (including attorney’s fees) arising from or related to my participation in the retreat.

This includes, but is not limited to, claims based on negligence, acts of God, accidents, allergic reactions, or third-party actions.

Force Majeure

I understand that Astralis Healing shall not be held responsible or liable for cancellations, delays, or changes due to circumstances beyond its control, including but not limited to natural disasters, pandemics, illness, strikes, or government restrictions.

Severability & Governing Law

If any part of this agreement is found invalid, the remainder shall remain enforceable. This agreement shall be governed by and construed in accordance with the laws of the State of North Carolina, where Astralis Healing operates.

Comprehensive Release of Liability

I, or my legal representative(s), fully release and hold harmless Astralis Healing, Jordyn Kurtz, all facilitators, assistants, contractors, venue owners, and associated personnel from any and all claims, damages, losses, injuries, or expenses arising from or connected to my participation in the retreat and all associated activities.

I acknowledge that I am participating voluntarily, assume full responsibility for my own health and safety, and understand that I do so at my own risk.

Acknowledgment of Understanding

By signing below, I confirm that I have read, understood, and voluntarily agree to the terms outlined in this waiver and consent form. I affirm that I am of sound mind and legal age to participate in this retreat.
